To become a member of the National Black Midwives Alliance, please fill out and submit this membership application form with your membership dues.

Please note that NBMA is an alliance committed to the advancement of Black midwives and Black student/aspiring midwives. Membership is open to self-identified Black midwives, self-identified Black student/aspiring midwives, and allies and organizations that support them. If you are not a self-identified Black midwife or Black student/aspiring midwife, then you must apply at the Ally or Organization membership level.
